Understanding Gastroesophageal Reflux Disease
Gastroesophageal reflux disease (GERD) is a chronic condition in which stomach contents flow back into the esophagus often enough to cause troublesome symptoms or damage. The lower esophageal sphincter, a ring of muscle that should close after food passes into the stomach, relaxes at the wrong times or has weakened. A hiatal hernia can make this worse.
GERD is common and very treatable. Left untreated, it can inflame the esophagus, cause narrowing, and in some people lead to Barrett’s esophagus, a change in the lining that raises cancer risk.
GERD Symptoms to Watch For
GERD symptoms range from typical heartburn to symptoms that seem unrelated to the stomach.
When to seek emergency care
Chest pain can come from the heart. Call 911 for chest pain with shortness of breath, sweating, or pain spreading to the arm, neck or jaw.
Symptoms of GERD
- Heartburn after meals or at night
- Regurgitation of sour liquid
- Chest discomfort
- Trouble swallowing
- Chronic cough or hoarseness
- Sour taste in the mouth
- Nausea after eating
These symptoms should be evaluated by a qualified medical professional.

The GERD Treatment Ladder
GERD treatment usually starts with simple steps and moves up only when needed.
Step 1: Lifestyle Changes
Earlier, smaller meals; raising the head of the bed; weight management; avoiding personal triggers; and stopping smoking.
Step 2: Medication
Antacids or H2 blockers for mild symptoms, and proton pump inhibitors for frequent symptoms or esophagitis, reviewed regularly.
Step 3: Testing and Procedures
Endoscopy and pH monitoring when symptoms persist, and anti-reflux procedures with a surgeon for selected patients.
What Makes GERD Worse?
Avoiding these common aggravating factors often reduces symptoms.
Late-Night Eating
Lying down within three hours of a meal lets acid flow back more easily.
Trigger Foods
Fatty or fried foods, chocolate, mint, citrus, tomato products and spicy food.
Alcohol and Caffeine
Both relax the valve and increase reflux for many people.
Smoking
Weakens the valve and reduces protective saliva.
When Should You Seek Care Promptly?
Some symptoms suggest complications and need prompt evaluation:- Difficulty or pain with swallowing
- Vomiting blood or black stools
- Unexplained weight loss
- Anemia
- Symptoms that continue despite daily medication
