Questions to Ask Your Insurance Carrier

Call the member services number on your insurance card before your visit or procedure. These questions help you understand what your plan covers.

Questions to ask before a gastroenterology visit or procedure
QuestionWhy it matters
Is Dr. Muhammad Ali Khan in network for my plan?In-network care usually costs less.
Do I need a referral to see a gastroenterologist? Some plans will not pay without a referral on file.
Does my procedure need prior authorization? Some plans require approval before endoscopy or colonoscopy.
What is my deductible, copay or coinsurance? These determine what you pay out of pocket.
Are the facility, anesthesia and pathology lab in network? They may bill separately from the physician.
Is my colonoscopy covered as screening or diagnostic?Screening and diagnostic exams can be covered differently.

Do I Need a Referral to See a Gastroenterologist?

Referral rules depend on your plan type. HMO and POS plans often require a referral from your primary care doctor before you see a specialist, while PPO plans generally do not. Many Medicare Advantage HMO plans also require referrals.

If your plan requires a referral, ask your primary care office to send it before your appointment. Physicians can find referral information on the physician referral page.

Before your first visit

  • Check whether your plan needs a referral
  • Confirm that Dr. Khan is in network
  • Have your insurance card and photo ID ready
  • Bring your medicine and allergy list
  • Bring copies of earlier test results

Questions? Call the office at (346) 340-4414.

Screening vs. Diagnostic Procedures and Coverage

Under the Affordable Care Act, most private plans must cover recommended colorectal cancer screening for adults 45 to 75 without cost-sharing, including a follow-up colonoscopy after a positive stool-based screening test. Medicare covers screening starting at age 45. A colonoscopy or endoscopy done to evaluate symptoms is usually billed as diagnostic, and your deductible or coinsurance may apply.

Questions About Your Coverage?

Call the office with your insurance information, and contact your insurance carrier to confirm benefits for your specific plan and service.

No Insurance or a High Deductible?

Patients without insurance, or with a high deductible, can ask about self-pay options. Uninsured and self-pay patients can request a Good Faith Estimate of expected charges for scheduled care under the federal No Surprises Act.
