Colonoscopy for women: screening consultation with a gastroenterologist

Colorectal cancer is the third most common cancer in women in the United States, yet many women put off colonoscopy because of worries about preparation, privacy or discomfort. Women also have some specific considerations, from symptoms that overlap with gynecologic problems to a colon anatomy that can make the procedure a little different. This guide from United Gastroenterology Associates explains colonoscopy for women, when to start, what to expect and how Dr. Muhammad Ali Khan cares for women in Katy and Cypress.

When should women get a colonoscopy?

Women at average risk should begin colon cancer screening at age 45, the same as men, and repeat a normal colonoscopy every 10 years. Women with a family history of colon cancer or advanced polyps, Lynch syndrome, inflammatory bowel disease, or a personal history of certain cancers may need to start earlier and repeat more often. Symptoms at any age should be checked.

Colonoscopy for women: what to know

  1. Start at 45Earlier with family history, Lynch syndrome or IBD.
  2. Symptoms overlapBloating, pelvic pain and fatigue are often blamed on other causes.
  3. Comfort and privacySedation, gowns and a respectful team make it easier.

Are colon cancer symptoms different in women?

The symptoms are the same, but in women they are often mistaken for other conditions. Fatigue from iron deficiency anemia may be blamed on heavy periods, bloating and pelvic discomfort on gynecologic causes, and bowel changes on IBS. Rectal bleeding may be attributed to hemorrhoids after pregnancy. Persistent symptoms deserve a colon evaluation, especially after 45.

Symptom Often blamed on Why it should be checked
Fatigue, low iron Heavy periods Can be from hidden GI bleeding, especially after menopause
Bloating Hormones, IBS Persistent bloating can have colon or ovarian causes
Rectal bleeding Hemorrhoids after pregnancy Polyps and cancer can also bleed
Bowel changes Stress, diet, IBS New changes after 45 need evaluation
Abdominal or pelvic pain Gynecologic problems Can come from the colon

Is colonoscopy different for women?

The procedure is the same, but women on average have a longer and more twisting colon, especially after pelvic or abdominal surgery such as a hysterectomy or C-section. This can make the procedure take slightly longer, but experienced gastroenterologists use techniques such as position changes and gentle pressure to complete it comfortably under sedation.

See a doctor promptly if you have

  • Iron deficiency anemia after menopause
  • A change in bowel habits lasting more than three weeks

Can you have a colonoscopy during your period or pregnancy?

Yes, a colonoscopy can be done during your period; use a tampon or menstrual cup and let the team know. During pregnancy, screening colonoscopy is usually postponed until after delivery, but a colonoscopy can be done safely if there is an urgent medical reason. Tell your doctor if you might be pregnant.

Tip: Some bowel preps can reduce the effectiveness of birth control pills taken around prep time. Use backup contraception for the rest of that cycle and ask your doctor if you have questions.

How do you prepare for a colonoscopy?

Preparation includes a low-residue diet for a day or two, clear liquids the day before, and a split-dose bowel prep. Arrange a ride home after sedation. See how to prepare for a colonoscopy and what you can eat before a colonoscopy.
